Here we report one case of lash grafting. This is a 22 years old male patient, who suffered from right eye symblepharon, cornea leucoma and both lids defect (average)c lashes loss, due to melting aluminum liquid burn 5 years ago. We released his symblepharon first, then did the upper lid lash grafting. The cilia were taken from mdial end of ipsilateral brow, sutured to a prepared upper lid recipient bed which was 2 mm away from the lid margin. The hair originally planted were lost gradually during the first 2 weeks. It took 2 months for cilia to regenerate. Postoperative 1 year follow up, showed the cilia grew well, and all in proper direction. The grafting of hair-bearing tissue is one of the least satisfying types of grafts. In the case of lash graft, the result is often an irregular array of sparse cilia growing in all directions. Instead of lash graft, we may use artificial eyelashes or tattooing the lid margin. However, it is unlikely that a male patient would avail himself of this artifice or tattooing. So sometimes, it is necessary to do a lash grafting. The best source of hair for lid lashes is the medial end of the eyebrow where the hairs are numerous. If’ these are unavailable for some reason, scalp hair from the occipital or temporal region may be used. The graft must be wide enough to include 4 rows of lashes (about 3 to 4 mm). This usually leases the 2 rows since the border lashes rarely survive surgery. Also the graft must be deep enough to include the root bulbs and this usually means a depth of about 3 to 3.5 mm.
