角膜移植後內皮細胞變化之研究

Endothelial Cells Loss and Morphologic Changes in Penetrating Keratoplasty

摘要


21個接受角膜移植的病人,沒有術後反應、青光眼、或任何手術的併發症。在術後2星期至2年間不定期接受內皮細胞攝影。分析術後7個月與22~24個月,內皮細胞密度與捐贈角膜之年齡或病人年齡沒有關係性存在。同時術後22~24個月之內皮細胞密度之差異性變小。手術後一年內細胞密度減少最大,一年到二年間細胞密度之變化漸趨穩定。21個病人接受角膜移植,其最佳矯正視力最小為0.05,視力0.1~0.5有9個,0.5以上11個。並從術後不同時期之內皮細胞攝影照片來探討角膜內皮細胞之癒合過程的型態變化。

21 cases of clear corneal transplants performed for various types of pathologic abnormalities were selected for study on the transplanted corneal endothelium. Cases with episodes of graft rejection, vitreous touch, and glaucoma after surgery were specifically excluded. The mean endothelial cells density postoperativdy in 3M, 12M, and 17-24M ere 1714, 1197 and 1067 cells/s. q.mm, but were not correlated with the age of the donor or recipient. It can be assumed that at the time the corneal transplant was performed the endothelial cell population on the transplanted donor tissue approximated that of the mean endothelial cell population of a normal adult cornea that is around 2500 cells/s. q.mm. That the mean endothelial cell loss at post op. 3M, 12M, and 17-24M. were 31%, 52, and 57%. These residual and enlarged cells seem to be able to compensate physiologically for the cell loss and to maintain clear, thin corneal grafts, and good vision. The healing process of damaged endothelial cells was discussed. The frequency of semistable cell constellations is correlated to disease activity in healing process of graft endothelium.
