摘要

用玻璃體切吸儀治療患眼前葉疾病的150人，169眼中，包括（一）織維化膜性白內障，59眼(34.8%)。（二）水晶體脫位或破碎32眼(18.9%)。（三）先天性白內障37眼(21.9%)。（四）瞳孔閉塞或黏連14眼(8.3%)。（五）發炎性瞳扎滲出膜9眼(5.3%)。（六）手術後殘留水晶體雜質6眼(3.6%)。（七）手術後玻璃體前黏連合併黃斑部囊樣水腫6眼(3.6%)。（八）殘遺增生性原始玻璃體2眼(1.2%)。（九）前房積血2眼(1.2%)。（十）無晶體性青光眼1眼(0.6%)。（十一）前房異物1眼(0.6%)。術後，患者病情及眼部外觀皆有改善，最終視力以Snellen chart改變二行以上為判定標準，視力進步者；76眼(53.7%)，視力不變者；60眼(42.8%)，視力減退者；5眼(3.5%)，因其他因素視力不能測定者28眼。
手術後併發症很少，包括（一）眼內出血14眼(8.3%)。（二）角膜內皮細胞代償缺失7眼(4.1%)。（三）視網膜剝離4眼(2.4%)。（四）水晶體殘留4眼(2.4%)。（五）虹膜意外傷害2眼(1.2%)。（六）脈絡膜剝離1眼(0.6%)。

Ocutome vitrectomy was used to treat 150 consecutive cases (169 eyes) with anterior segment lesions. The lesions included: (1) Fibrotic capsulolenticular membrane in 59 eyes (34.8%) (2) Subluxated or dislocated lens in 32 eyes (18.9%) (3) Congenital cataract in 37 eyes (21.9%) (4) Occluded and updrawn pupil in 14 eyes (8.3%) (5) Exudative pupillary membrane in 9 eyes (5.3%) (6) Retained lens material in 6 eyes (3.6%) (7) Anterior vitreous touch with cystoid macular edema in 6 eyes (3.6%) (8) PHPV in 2 eyes (1.2%) (9) Total hyphema in 2 eyes (1.2%) (10) Aphakic glaucoma in 1 eye (0.6%) (11) Foreign body in anterior chamber in 1 eye (0.6%). Most of the abnormal tissues were removed with excellent postoperative status of anterior segment. Vision improved at least two lines on the Snellen chart in 76 eyes (53.7%). Vision unchanged in 60 eyes (42.8%). Vision was worse in 5 eyes (3.5%).
The mean follow up period was 12.4 months. Complications were rare, including (1) Intraocular hemorrhage 14 eyes (8.3%) (2) Corneal decompensation in 7 eyes (4.1%) (3) Later retinal detachment in 4 eyes (2.4%) (4) Residual lens material in 4 eyes (2.4%) (5) Accidental iridectomy in 2 eyes (1.2%) (6) Choroidal detachment in 1 eye (0.6%).
There were 28 eyes which vision could not be detected postoperatively due to the young age, and the reasons for unchangable vision were (1) Amblyopia (2) Secondary glaucoma (3) Macular scar or hole (4) Old retinal detachment (5) Optic nerve atrophy.
