摘要

由Histoplasma capsulalum感染所引起的疑似眼部組織漿菌病症候群，東方人很少見，尤其是還合併有視神經乳頭病變的更少。近來，作者發現一病例，除了有典型的POHS眼底所見外，還有視神經盤水腫、發炎等現象。在整個病程中，抗組織漿菌素抗體效價從陰性轉為陽性；服用類固醇藥物，可改善病情。由於此病例合併視乳頭炎和前葡葡膜炎，故提出報告。

We presented a case of active POHS with optic nerve head involvement. In this case, we also found circumpapillary choroiditis, paramacular lesion and equatorial histo spots, anterior uveitis and swollen optic disc. The anti-histoplasmin antibody titer changed from negative to positive (1:16) during the course. Systemic steroids improved the vision and suppressed the ocular inflammatory changes.
On the basis of mild impairment of color sense and generalized constriction of visual fields with enlarged blind spots, the authors supposed that the inflammatory change of the disc was some form of papillitis. If this is the case, it is still controversial whether this papillitis is primary in origin or secondary to circumpapillary choroiditis.
