誤診為篩狀竇膿囊腫的眼窩平滑肌瘤症例報告

摘要

眼窩平滑肌瘤在眼窩腫瘤中可說是非常罕見的一種腫瘤，在世界文獻上到目前僅有12個病例報告，它的特性為使眼球慢慢突出，對視機能之影響少，組織學上可見到排列整齊或成漩渦狀排列的細胞，其大小，形狀相當一致。因為病例少，所以常被誤診為其他腫瘤，本文所報告的一例是臺大眼科第一個眼窩平滑肌瘤，手術前也被誤診為篩狀竇膿囊腫，經手術完全取出腫瘤後，作病理切片檢查才知道是平滑肌瘤。

This paper is to report a 25 year-old male patient who was previously misdiagnosed as ethmoid pyocele and thereafter proved to be orbital leiomyoma pathologically.
The orbital leiomyoma is very rare in incidence and the tumors were thought to be originated from various tissues as follows: 1) Müller muscle, 2) Hesser muscle, 3) vascular tissue.
The clinical picture is not specific and the diagnosis can not be made without pathology.
Complete excision is the principle of treatment. Due to its rare possibility of malignancy change, the prognosis is rather good.
It is very easy to be misdiagnosed clinically and the reason of misdiagnosis on this patient is fully discussed in this paper.
