矽油之使用對嚴重視網膜剝離之治療

本篇報告使用玻璃體切除術並於眼內注入矽油之方法來治療各種嚴重複雜型視網膜剝離之經驗。總共治療病例28位病人28隻眼睛，包括增殖性玻璃體視網膜病變(PVR)之D2和D3度，後極部裂孔，巨型裂孔，反覆再發性視網膜剝離以及增殖性糖尿病性視網膜病變併發嚴重牽引性視網膜剝離。追踪期為三個月到一年。結果顯示解剖上成功者54%，失敗46%，視力進步者32%，不變者29%，惡化者39%。較重要之合併症有角膜帶狀病變4例，內白障3例，新裂孔形成4例。著者結論矽油對嚴重視網膜剝離之手術確有幫助，在妥當的使用下其合併症並不十分嚴重，但矽油之效用卻也有其相當程度之限制。

Twenty-eight cases of complicated retinal detachment were treated with an intracameral silicone oil injection following a pars plana vitrectomy procedure. These 28 cases comprised proliferative vitreoretinopathy of D2 and D3 degrees, retinal detachments with posterior breaks, giant retinal tears, unexplained repeated recurrent detachment and proliferative diabetic retinopathy complicated with thick pre-retinal fibrous tissue and traction retinal detachment. These cases were followed for three months to one year. The results showed anatomical success in 54% and failure in 46%. There was an improvement of vision in 32%, no change in 29%, a worsening in 39%. The serious complications included band keratopathy in 4 cases, cataracts related to silicone oil in 3 cases, new breaks in 4 cases. The authors conclude that silicone oil is a helpful and efficacious adjunct in the management of complicated retinal detachment. The complications are not so severe if used appropriately. However, it is also found that silicone oil certainly has its limitations for those cases with severe retinal shortening and contraction.
