一位44歲男性於白障摘出後併發細菌性眼內炎,經反覆之抗生素眼球內注射消退,卻於手術後兩個半月併發真菌性眼內炎,培養顯示是由Penicillium菌屬所引起。本例經後玻璃體切除術合併Amphotericih B及Miconazole眼球內注射後痊癒,最後追踪視力為0.1。
Although penicillium is a common environmental fungus and frequently found in the normal conjunctiva, but incidences of proven endophthalmitis due to this fungus were rare. A 44 years old male patient had developed bacterial endophthalmitis after the cataract extraction. The infection was quieten soon after repeated intraocular antibiotics injections. Two and half months after the surgery, he then suffered from another attack of endophthalmitis. Penicillium species was cultured from vitreous aspirates in the Sabouraud's broth. Posterior vitrectomy and intraocular injection of antifungal agents including amphotericin B and miconazole were effective in this case. No further evidence of relapse was observed after follow up for an 11 months period. The final visual acuity of the patient was 20/200. We believed a combination of early posterior vitrectomy with introcular injection of antifungal agents are effective in treatment of fungal endophthalmitis.
