本篇報告一例視網膜胚母細胞瘤自發性消退的病例,由於病人在漿液性剝離的視網膜下可見透明而不規則的腫瘤呈乳酪狀的鈣化及腫瘤周邊的為網膜色素層細胞的變性等。加上病人過去史中無放射線及藥物治療的記錄及染色體上有13q14脫落的現象,可以證實為一例自發性消退的視網膜胚母細胞瘤。
Retinoblastoma is the most common intraocular malignancy that occurs during childhood. The incidence of this tumor is approximately one in 14,000 to 20,000 live births. To our knowledge, about 1% of retinoblastoma cases tend toward spontaneous regression. A 21 year old female patient presented spontaneous regression of retinoblastoma in her right eye, evidenced by a Cottage-cheese pattern of residual tumor with marked calcification at the lower quadrant of the fundus. The surrounding retina revealed a chorioretinal scar with retinal pigment epithelium degeneration. At the age of 1, her left eye was suspected of having a malignant intraocular tumor and was enucleated. The authors also discussed the possible mechanism of spontaneous regression of retinoblastoma. We emphasize that a mutation of a tumor cell occuring at later stages of cellular development would result in a benign tumor; but if the cellular mutation occurs at earlier stages of blast cell, the tumor would be malignant.
