囊外晶體摘除及後囊人工水晶體植入術後角膜水腫歷程與內皮細胞之關係

The Correlation between the Natural History of Corneal Edema and Endothelial Cell Loss after Ecce & Posterior Chamber IOL

摘要


105例單純性老年白內障患者,在施以單眼囊外晶體摘除及後房人工水晶體植入術之前後,皆接受超音波角膜厚度及內皮細胞數之測定。結果發現中央角膜厚度平均為0.520±0.033mm,內皮細胞數平均為2122.3±197.5/平方公厘;手術後中央角膜快速變厚,並於術後第三天達到頂點,約需二個月角膜才恢復到原有之厚度;手術眼之內皮細胞喪失7.73%,而沒開刀之眼睛則喪失2.25%,無法以術後角膜變厚之程度來推測內皮細胞喪失率。手術後之眼睛不經矯正視力好於或等於6/12者佔95.3%,若經矯正則97.1%之眼睛視力好於或等於6/7.5。囊外晶體摘除及後房人工水晶體植入術是一種安全且有效的白內障治療方法。

Consecutive series of 105 non-complicated cataracts underwent unilateral extracapsular cataract extraction and posterior chamber intraocular lens implantation smoothly are studied. The central corneal thickness and endothelial cell count were measured on both eyes pre- and post-operatively. The mean value of central corneal thickness is 0.520 ± 0.033mm. And the mean value of the endothelial cell count is 2122.3±197.5/mm^2. It takes about 2 months before the corneal restores the thickness. During the follow up period (6.8 months), the mean of the endothelial cell loss of the operative eye is 7.7%, and the fellow non-operative eye is 2.25%. There is no relationship between the postoperative changes of the central corneal thickness and the endothelial cell loss (P>0.05). 95.3% of the operated eyes, the visual acuity were better than 6/12 without any correction, and if corrected, 97.1% the visual acuity were better than 6/7.5. Extracapsular cataract extraction and posterior chamber lens technology is unrivaled with safety and efficacy.
