以穿透虹彩之角鞏膜後房人工水晶體固定術應用於白內障囊外摘除手術時發生後囊巨大破裂時完成人工水晶體之植入
Trans Iris Comeo-Scleral Fixation of PC-IOL in the Cases of Large Posterior Capsule Repture during Cataract Surgery
摘要
近幾年來，國內外多位醫師前輩曾提出各種後房人工水晶體的植入技術，應用於無後囊或後囊巨大破裂的術眼。本院眼科參考其方式併作一些技術上的修改，稱為「穿透虹彩之角鞏膜後房人工水晶體固定術」，應用於白內障囊外摘除手術時，發生後囊巨大破裂併玻逸出情況下，以一次手術方式，同時完成白內障摘除合併後房人工水晶體植入，代替目前常用的前房人工水晶體植入術。於1989年6月間，三位雙眼白內障住院病人，在院接受手術治療，一眼接受本手術方式，另一眼則接受順利的傳統白內障摘除手術合併後房人工水晶體植入。術後結果令人滿意，且無特殊併發症發生。本文將手術方式及過程和術後結果作探討，併和其他前輩醫師的手術方式作討論比較。

Several authors have recently described their techniques for implantation of PC-IOL in cases of absence of the posterior capsule. We modified some methods and implanted PC-IOL immediately after management of vitreous loss in the cases of large posterior capsule rupture during cataract surgery, instead of AC-IOL implantation. The method we called is ”Trans Iris Corneo-scleral Fixation.” We will discuss the difference between cases of ours and theirs, and some problems concerning our procedure will also be discussed.
