角膜切片在慢性角膜炎的診斷價值
The Diagnostic Value of Corneal Biopsy in Chronic Keratitis
摘要
慢性角膜炎的診斷，很多時候不能單靠臨床表徵來決定；尤其是黴菌、非典型結核菌及阿米巴蟲所引起的感染，致病體常侵入至角膜基質的深部，此時單靠角膜抹片及培養可能無法提供答案，而必須利用角膜切片來作為輔助的診斷工具了，本報告中所提出的七個慢性角膜炎病例，其中三例是黴菌感染，四例是非典型結核菌感染，皆是重覆作了幾次角膜抹片及培養均屬陰性反應，最後才靠角膜切片確定診斷的。本篇即就角膜切片之診斷價值，作一概括之討論。

The diagnosis of chronic keratitis poses a difficult problem for the ophthalmologist. In many cases it cannot be based on the clinical manifestations alone, and corneal smears and cultures are mandatory. The latter procedures, however, may fail to reveal the etiological agent in deep infections such as fungal, atypical TB, or amoeba keratitis. Corneal biopsy will then be a useful diagnostic adjunct in such circumstances. This report includes 7 cases of chronic keratitis which were initially undiagnosed by repeated corneal scrapings and cultures. Three cases were fungus infections while the remaining 4 cases were proved to be atypical TB keratitis.
