近視性前角晶體式形術矯正深度近視之效果

摘要

深度近視(Excessive myopia)之病患，不論以眼鏡或隱形眼鏡來矯正，常常無法達到令人滿意之效果，尤以兩眼度數差異過巨時為甚。本文討論近視性前角膜晶體成形術(Myopic epikeratophakia)對矯正深度近視之效果。並報告一病例。病人右眼自小即患深度近視，屈光檢查為相當球面度數－20.5D，祼視力為0.02。接受前角膜晶體整形術後經過良好，術後一年四個月檢查祼視力0.2，屈光狀達相當球面度數+2.25D，並可以矯正到0.4。此項手術甚為簡易和安全，對於深度近視且無法適應隱形眼鏡者，值得考應施行。

The correction of excessive myopia has been a challenge to ophthalmologist in case of contact lens intolerance. Clear lens extraction is an old method but the surgen has to face the risk of retinal detachment and cystoid macular edema. Recently epikeratophakia has been found quite effective and safe to correct myopia. We report an adult male patient who had excessive myopia in his right eye with refractive error of -20.5 D in spherical equivalent. Myopic epikeratophakia was performed in late 1985. Examination sixteen months after surgery the graft remained clear and his vision had improved to 0.2 and can be corrected to 0.4 with +2.25 D lens. As its relative simplicity and safety, myopic epikeratophakia is an useful alternative for the correction of excessive myopia.
