慢性硬化性眼窩偽腫瘤之兩例報告

摘要

一般之眼窩偽腫瘤(orbital pseudotumor)，其病理變化以廣泛之淋巴球浸潤為最常見，但以全般性纖維組織浸潤為主要病理型態之慢性硬化性眼窩偽腫瘤(sclerosing orbital pseudotumor)則較為罕見。
本文報告二例慢性硬化性眼窩偽腫瘤；此二例皆經神經外科做球後組織取樣及病理切片證實。二例主要臨床症狀為眼球凸出，眼球運動障礙及視力喪失；電腦斷層攝影皆顯現球後腫塊；二例皆投予類固醇藥物及接受放射線治療，但都沒有良好之反應。

Sclerosing orbital pseudotumor, a particular type of inflammatory process, is diferrent from acute orbital pscudotumor both in pathological and clinical aspects. We present two patients who underwent orbital exploration yielding the diagnosis of sclerosing orbital psuedotumor. The presenting symptomes were exophthalmos, frozen eye ball and visual loss. Computed tomographic scans showed solid uniform retrobulbar mass with intracranial extension, bone extension and paranasal sinus extension. Steroid and radiotherapy were ineffective. Surgical intervention allowed correct histological diagnosis when idiopathic proptosis was associated with progressive visual loss and ineffectiveness to steroid and radiotherapy.
