急性視網膜壞死合併非菌性腦膜腦炎－報告一例疑似延續性擴散

摘要

本文報告一例急性視網膜壞死合併非菌性腦膜腦炎疑似延續性擴散。一31歲健康男性，在感冒微發燒後，開始神智不清，經神經內科診斷為非菌性腦膜腦炎，並給予支持性治療後，病情穩定。但右眼紅、痛、視力從發病前的1.0，而於急性發炎期減退為無光感；經投予局部及全身性類固醇藥物治療。視力一度明顯改善，後因視網膜繼續壞死、剝離，影響黃斑部，使視力最後只剩下手動感。

We report a case of acute retinal necrosis (ARN) associated with aseptic meningoencephalitis on the basis of presumed contiguous spread. A 31-year-old healthy man developed an upper respiratory infection follow by conscious disturbance in mid May 1987. After diagnosed aseptic meningoencephalitis by Neurologist, he was admitted to NICU, after one week his general condition was stable with supportive treatment. However, his visual acuity 1.0 of both eyes before illness was known to deteriorate to no light perception for his right eye, while left eye remained normal, by using topical and systemic steroid treatment, vision of his right eye improved abruptly. But was finally sad with massive retinal necrosis, detachment and outcome with only handmovement for his right eye.
