摘要

本篇報告一例因外傷引起眼內炎，經給予玻璃體內抗生素注射(I. V. I.)，眼內炎雖得以控制，却發生視網膜毒性反應(retinal toxic reaction)。視網膜毒性包括：棉絮樣滲出，視網膜水腫，視網膜出血等缺血性變化；晚期造成視神經乳頭泛白萎縮及視網膜色素層變性。

Antibiotic intraocular injection is indeed a very effective therapeutic method for endophthalmitis, but it also must be very carefully administered to prevent antibiotic induced toxicity to retina and other intraocular tissues. Retinal toxicity is most important for it frequently result in retinal ischemic changes: cotton-wool like exudative change, retinal edema and retinal hemorrhage which in turn can cause some irreversible changes like retinal pigmentory degeneration and optic nerve atrophy finally.
This report is a case of post-traumatic induced endophthalmitis. After intraocular antibiotic injection, retinal toxicity had occured despite that endophthalmitis was put under control.
Therefore, the selection of proper nontoxic dosage, careful preparation & dilution of antibiotic and proper method of injection: the site of injection, direction of the needle bevel and speed of injection are all important factors when intraocular antibiotic injection was administered.
