非接觸型氣體平壓式眼壓計之臨床評估

Clinical Evaluation of the Non-Contact Applanation Tonometer

摘要


利用脈衝氣流測量眼壓,機器可以不必接觸到角膜。但這類氣體平壓式眼壓計,早期的設計,病人必須坐著採固定的姿勢才能測量。而經改良的手持式氣體平壓式眼壓計-Pulsair,在測量方法上較為簡便。本文使用Pulsair測量眼壓。對角膜正常的病人,以Goldmann平壓式眼壓計之測量值為標準,結果發現眼壓30mmHg以下時,Pulsair和Goldmann平壓式眼壓計之測量值相當接近(y=0.96x+1.51, S.D.=1.73);而眼壓在30mmHg以上時,二者之差異明顯變大(y=0.69x+12.40, S.D.=3.08)。對角膜不正常的病人,以Pneumotonometer和Pulsair之测量值做比較,差異也較大(y=0.91x+0.21, S.D.=3.35)。

A new type of non-contact tonometer (Pulsair) is introduced. It is handheld with the advantages of portability and insensitivity to orientation. We evaluate it in the clinical trials. When the IOP below 30 mmHg, the paired Pulsair and Glodmann readings show close agreement. (r=0.93, Y=0.96Z+1.51, S.D.=1.73) However, if the IOP is above 30 mmHg, their difference increases. (r=0.91, Y=0.69X+12.40, S.D.=3.08) For the patient with abnormal cornea, comparative IOP measurements between Pulsair and Pneumotonometer also showed greater difference. (r=0.87, Y=0.91X+0.21, S.D.=3.35) Besides, the readings of Pulsair have more variability than those of the Goldmann tonometer. Great caution should be taken on the above conditions.
