先天性汗性外胚層發育不良症
Congenital Hidrotic Ectodermal Dysplasia
摘要
汗性外胚層發育不良症具眼睛表徵是很罕見的疾病。本文即報告一汗性外胚層發育不良之病例且具眼睛之異常。患者為18歲男性，主訴為兩眼燒灼感已有一月。除全身有典型外胚層發育不良變化外，局部可見角膜表皮下浸潤及新生血管形成。於其手足皮膚切片可見表皮過度角化，但可見汗腺之存在；且其排汗試驗顯示汗液並無減少，向此可與無汗性作鑑別診斷，Schirmer's test淚液分級並無減少，故可推論其角膜變化可能是一種原發性角膜缺損所造成，並非單純向於淚液分泌減少之故。目前病患治療以人工淚液及油膏防止角膜狀況惡化，情況維持穩定。

Congenital hidrotic ectodermal dysplasia (H.E.D.) is a rare inherited disease, which manifests itself with hyperkeratosis of the palms and soles, nail malformation and hair loss. We report in here a case of H.E.D. with typical clinical manifestations and comeal defects. With the aids of skin and bulbar conjunctival biopsies, schirmer's test, and iodine-starch sweating test, we suggest the possible pathogenesis of corneal changes may be the primary defect of H.E.D.
