複雜性視網膜剝離合併嚴重增殖性玻璃體視網膜病變的手術處理
Surgical Management of Complicated Retinal Detachment with Severe Proliferative Vitreoretinopathy
摘要
增殖性玻璃體視網膜病變(PVR)是現網膜復位手術失敗的主因。我們報告15例PVR C3以上的病例接受手術治療的結果。手術技術包括玻璃體切除術，網膜上纖維膜剝除，網膜切開或切除，內引流，眼球內光凝固，矽油填充，鞏膜環扣等，其中6例視網膜完全復往，3例黃斑部復位，6例失敗。6例完全復位中有5例視力進步或穩定，1例退步。

Proliferative vitreoretinopathy is the leading cause of failure in retinal reattachment surgery. We reported a series of 15 cases of complicated RD with severe PVR treated by vitrectomy and supplemental procedures including endophotocoagulation, silicone oil tamponade, membrane peeling, retinectomy etc. The result is that 6 cases are completely reattached, 6 cases are considered failure, and only macular reattachment is found in another 3 cases. In 5 of the 6 successful cases, visual acuity are improved or stable, the rest become worse.
