信封式及閉罐式囊外白內障摘除併後房人工水晶體植入術後角膜內皮細胞密度之比較（回溯性研究）

Comparison of Endothelial Cell Density Following Extracapsular Cataract Extraction & Posterior Chamber Intraocular Lens Implantation between Endocapsular and Can-Opener Technique

摘要

我們收集了101個眼睛，均接受囊外白內障摘除，同時植入後房人工水晶體的手術。其中有45個眼睛採用信封式術式，另外56個眼睛採用傳統開罐式術式。術後追蹤時間分別超過七個月及十五個月。我們以角膜內皮細胞顯微鏡評估術後角膜內皮細胞密度。信封式這組病人平均為1941±384/平方公厘，而開罐式則為1670±552/平方公厘。平均信封式高出開罐式271/平方公厘。
在信封式這一組，由於手術操作時，均有幾近完整的前囊膜，可以保護著角膜內皮細胞免於被灌輸液之衝擊，因此，角膜內皮細胞的損失較少。

We collected 101 eyes receiving ECCE & PCIOL implantation, 45 of them with endocapsular technique and the other 56 eyes with can- opener technique. Postoperative endothelial cell density was evaluated with specular microscope. The endocapsular group had an average endothelial cell density of 1941±384 cells/mm^2, while the can-cpener group had 1670±552 cells/mm^2. The endothelial cell density of the endocapsular group was higher than that of the can- opener group by 271 cells/mm^2.
In the endocapsular group, the corneal endothelium could be better protected by the almost intact anterior capsule during manipulation. In this point of view, it was concluded that the endocapsular technique was superior to the can-opener technique.
