Argon Laser Trabeculoplasty for Chronic Angle-Closure Glaucoma Uncontrolled by Laser Iridotomy

From Mar. 1987 to Feb. 1990, 19 patients (20 eyes) with chronic angle closure glaucoma underwent argon laser trabeculoplasty (ALTP) 1 month-33 months after laser iridotomy due to uncontrollable lop with maximal tolerable antiglaucoma medication. 15 eyes (75%) were successful within 6 months after ALTP whose IOP remained less than 20 mmHg and no advance in VF defect and optic nerve damage. 5 eyes (25%) were failed within 6 months after ALTP, 2 eyes were failed within 1 month, 2 eyes within 1-3 months and 1 eye within 3-6 months. In successful group, the mean IOP before ALTP was 24.13±3.60 mmHg and mean IOP 1 month, 3 months, and 6 months after ALTP were 13.67±2.32 mmHg, 13.00±3.56 mmHg, and 13.60±2.59 mmHg. The mean reduction of IOP 6 months after ALTP was between 10.5 mmHg and 11.1 mmHg. There was statistic difference in reduction of IOP before and after ALTP. The success rate and mean reduction of IOP 6 months after ALTP were not associated with age of patient, area of PAS before ALTP and number of laser burn of ALTP. Patients with higher pretreatment IOP (>24mmHg) had greater mean IOP reduction after ALTP, but, lower success rate. ALTP can be tried, before filtering operation, in patients with CACG whose IOPs were poorly controlled with maximal tolerable antiglaucoma medication after LI.
