急性前葡萄膜炎合併自發性前房出血－病例報告
Acute Anterior Uveitis Complicated by Spontaneous Hyphema-Cases Report
摘要
我們提出兩例因急性前葡萄膜炎併發自發性前房出血的病例報告。兩例均為年輕人，一為男性，一為女性。兩例均無眼球外傷病史，且眼科學檢查均無虹膜新生血管、幼年型黃色肉芽腫或者疱疹病毒感染等異常發現。 免疫學檢查發現兩例均有HLA-B27抗原，且女性病人同時有類風濕因子的陽性反應。 兩例均先接受類固醇及睫狀肌鬆弛劑等保守療法。其中一例因保守療法效果不佳，前房血塊吸收緩慢且發炎一直非常厲害，因而作前房的沖吸治療及前房水之細胞學診斷，追蹤至今超過一年以上，均無復發之跡象。

We reported two cases of acute anterior uveitis complicated by spontaneous hyphema. One was a young lady and the other was a gentleman. No trauma history could be elicited. Ocular examinations revealed no pertinent findings, such as rubeosis irides, juvenile xanthogranuloma, or Herpes infections. Immunological studies disclosed HLA-B27 antigen present in both patients and positive RA factor in the lady. Initially, conservative treatment with steroid and cycloplegics was given to both patients. Surgical intervention with I/A to remove blood clot was added to that gentleman due to slow resorption of blood clot and strong reaction in the anterior chamber. Both patients ended in clear and quiet anterior chamber without recurrence for more than one year.
