小樑切除術後之巨大非猛爆性脈絡膜上出血―病例报告
Massive Nonexpulsive Suprachoroidal Hemorrhage Following Trabeculectomy-Case Report
摘要
x遲發性非猛爆脈絡膜上出血是眼内手術後少见的併發症。我们报告一例次登性隅角阅放性青光眼患者，接受小樑切除術後第三天發生環状浆液性脈络膜剝離，隔天又發生脈络膜上出血，造成親吻之脈絡膜剝離。當天即由鞏膜後切開術進行引流，但效果極微。四天後第二次引流，终於使脈絡膜復位，並回復術前之視力。不幸地，又於一週後發生裂孔性视綱膜剝離，最後以鞏膜環扣術和玻璃體切除術成功地使视綱膜復位。有關此一嚴重併發症的危險因子、致病機轉、影像检查，及目前之治療原則在本文均有討論。

Delayed nonexpulsive suprachoroidal hemorrhage is a rare complication following intraocular surgery. We present a case of secondary open angle glaucoma who underwent a uneventful trabeculectomy. Annular serous choroidal detachment was noted on the third postoperative day. Secondary hemorrhage resulted in a kissing detachment on the fourth postoperative day. Surgical drainage through posterior sclerotomy on the first day proved to be ineffective. Secondary drainage four days tater resulted in resolution of the detachment and restored preoperative vision. Subsequent rhegmatogenous retinal detachment one week tater was successfully managed by scleral buckling and vitrectomy. The risk factors, pathogenesis, image study, and current management of this severe complication are reviewed.
