梅毒性神經視網膜炎－臨床及電生理學之一例觀察報告
Syphilitic Neuroretinitis-A Case Report of Clinical and Electrophysiological Study
摘要
我們記錄了一位患有梅毒性神經視網膜炎的42歲男子在接受治療前後PVEP和PERG的變化。在治療前，兩眼PVEP P100波的出現時間輕度延後，但PERG P50及N95波的振幅明顯地減弱甚至消失。隨著治療後監床症狀的改善，這些波也慢慢恢復正常。顯示在病變早期視神經以及整個視綱膜都受到波及，而經過藥物治療之後，病情漸漸好轉，避免了節神經細胞進一步的損傷和視神經的脫髓鞘病變、這個結果和我們臨床上的觀察是一致的。

A 40-year-old healthy man, 2 months after partial recovery of sudden visual loss in the left eye, developed painless bilateral neuroretinitis, The simultaneous recording of his pattern VEP and pattern ERG revealed absent pattern ERG with minimal delayed pattern VEP. The FAG study of his retina showed delayed retinal arterial filling and increased capillary perfusion wills late staining of the disc. The serology of his blood and spinal fluid disclosed neurosyphilis. After intravenous penicillin G and oral steroid treatment for 7 days, his vision improved with recovery of pattern ERG recording. From this case, we demonstrated that pattern ERG reflected inner retinal layer dysfunction of syphilitic neuroretinitis and future application is encouraged.
