Syphilitic Episcleritis-A Case Report

摘要

上鞏膜炎可能是全身性疾病的初癥兆，雖然有明顯因果關係者約10%，但有一半伴隨著全身性疾病。本篇報告一位六十歲男性病人泛發兩眼性上鞏膜炎，最初給予類固醇治療反應良好，但數週後復發，且對類固醇治療反應出現頑抗。經過進一步的全身性抽血及X光檢查，顯示梅毒檢查(VDRL & TPHA)為陽性反應，但並沒有其他梅毒的身體表現出現。經過連續三週每週二百四十萬單位的盤尼西林(Benzathine penicillin)肌肉下注射且無類固醇藥物給予，上鞏膜炎症狀逐漸緩解，經追踪檢查一年後並無復發。

Episcleritis may be the initial sign of a systemic disorder, although less then 10% of the patients with episcleritis have an apparent predisposing condition. We report a 60-year-old male patient who developed bilateral diffuse episcleritis. Initial response to corticosteroid therapy was good, but several weeks later, it recurred with poor response to steroid treatment. After series of further study, a positive VDRL test and a reactive TPHA test were noted. No other signs of syphilis were apparent. We started treatment with Benzathine penicillin (2.4 million units intramuscular per week × 3 weeks) without steroid eye drop supplement and his episcleritis resolved gradually. Syphilitic episcleritis was diagnosed.
