腎上腺嗜鉻細胞瘤合併兩側高血壓視神經病變－病例報告
Adrenal Pheochromocytoma with Bilateral Hypertensive Optic Neuropathy-Case Report
摘要
28歲年輕男性由於頭痛，視力模糊前來求診，眼底鏡檢查發現兩側嚴重視神經盤水腫、出血以及黃斑部星狀滲出液(macular star)。右眼視力20/40，左眼視力僅存指數。病人因疑似腦瘤住院接受進一步檢查。於住院期間，發現血壓有明顯上升現象(240/160mmHg)。尿液中Vanillylmandelic acid含量顯著增加(79.5mg/24hrs)。腹部超音波及電腦斷層掃瞄顯示右側腎上腺腫瘤。經會診泌尿外科手術切除腫瘤後，血壓回復正常，視力進步到兩眼20/20。病理報告證實為嗜鉻細胞瘤(pheochromocytoma)。

A twenty eight years old male complained of frequent attack of transient obscuration of vision was found to have blurred disc margin with hard exudate, flame-shaped hemorrhage over posterior pole and macular star bilaterally. The visual acuity was 20/40 on the right eye and counting finger/20cm on the left eye. The patient was admitted for further management and evaluations under the impression of intracanial mass lesion. The computed tomography of the brain was normal. Immediately alter admission, elevated blood pressure was noted (240/160 mmHg). The sonography and computed tomography of the abdomen showed a right suprarenal mass. After total removal of the tumor, the blood pressure returned to normal and the visual acuity spontaneously improved to 20/20 on both eyes. The pathology showed typical pheochromocytoma pattern under microscope.
