Diode Laser Photocoagulation for Prethreshold Zone I Retinopathy of Prematurity-Case Report

摘要

出現於視網膜第一區之早產兒視網膜病變為一嚴重之疾病。在疾病到達threshold時冷凍治療之效果遠低於出現於周邊網膜之病變。許多專家建議治療時機宜提早。我們以二極體雷射治療一位非常早產之嬰兒（出生週數23週，出生體重650公克）兩眼眼底均有第一區不及threshold之早產兒視網膜病變，左眼病有”plus”徵候。在局部點眼麻醉下，病患接受兩眼廣大無血管區之間接眼底鏡之二極體雷射治療。早產兒視網膜症在追踪九個月後呈現退行變化，並有良好之後極部解剖情況。此臨床經驗顯示光凝固對第一區之早產兒視網膜病變可能有積極之療效。

Retinopathy of prematurity involving zone I of the retina is a serious disease. Cryotherpy is far less effective to treat threshold zone I disease than the more anterior counterpart. Several authors have suggested early treatment for this condition. We used diode laser to treat one extremely premature infant (gestational age 23 weeks, birth weight 650 gm) who had bilateral zone I disease with less than threshold severity. Plus sign was noted in one eye. Under topical anesthesia, photocoagulation delivered through an indirect ophthalmoscope was placed in the widespread avascular area in both eyes in the neonatal intensive care unit. Regression of retinopathy of prematurity with good anatomical outcome was noted after nine months' follow-up. Early intervention with laser photocoagulation may be helpful in treating zone I retinopathy of prematurity.
