帶狀疱疹眼症併發綠膿桿菌眼內炎
Varicella-Zoster Ophthalmicus Complicated with Pseudomonas Aeruginosa Endophthalmitis-Case Report
摘要
一名62歲男性病人因左側帶狀疱疹眼症至本院眼科求診。經檢查發現：沿著左側三叉神經眼分枝分布之皮膚，包括鼻尖部分，出現大小不一的水泡痕跡，眼瞼亦水腫下垂，角膜上皮點狀發炎，角膜沈澱物和前房細胞且左眼的外展運動完全受限。經住院給予抗病毒Zovirax藥膏及副腎皮質荷爾蒙點眼藥治療後，獲得改善。出院後第三天，左眼忽然出現大量黃色黏性膿樣分泌物，角膜壞死混濁，前房蓄膿及玻璃體緻密膜狀物。經細菌培養證實為綠膿桿菌感染，並給予三星期的抗生素治療後，眼部症狀終獲緩解。因此病例頗為罕見，故提出報告。

A 62-year-old man presented with ophthalmic herpes zoster with multiple variable size of vesicles along the dermatome of left ophthalmic branch of trigeminal nerve and ptotic eyelid. Slit lamp examination revealed epithelial keratitis, keratic precipitation and reactive cells in anterior chamber. Complete limitation of left ocular abduction movement was noted. After antiviral Zovirax ointment and tepaca1 1% Dexamethasone treatment, the ocular symptoms subsided. Later, massive mucopurulent discharge, corneal necrosis and hypopyon of left eye developed gradualy; the bacterial culture demostrated Pseudomonas aeruginosa. Therefore the topical and systemic antibiotics were instituted for three more weeks with response.
