Blood-Retinal Barrier Change of Hypotony Retinopathy Late after Trabeculectomy with Mitomycin-C Soaking

小樑網切除術後之晚期併發症:低眼壓視網膜病變-患者網膜血管屏障變化之研究

摘要


為了研究低眼壓視網膜病變患者網膜血管屏障之變化,作者統計了該院過去四年間接受青光眼小樑網切除術後,晚期產生低眼壓視網膜病變的患者,經篩選後,總共收集了六位病例,七隻患眼,作者發現這些病患在手術時均有使用局部Mitomycin-C作鞏膜浸潤。而產生低眼壓視網膜病變的時間至少距離手術四個月以上(四至參拾玖個月),以避免手術本身引起的反應會影響網膜血管屏障。研究的方法是以玻璃體螢光測量儀(Fluorotron Master @)加上標準的網膜螢光血管攝影為主。結果顯示在眼底表現為網膜血管扭曲,脈絡膜網膜縐褶,甚至於黃斑部水腫狀變化的患者,其網膜血管屏障都是正常的。但在一位雙側眼底呈明顯視神經盤水腫的患者,其螢光血管攝影與玻璃體螢光測量儀均顯示出不正常的高螢光表現。由於視神經盤血液供應來源為後睫狀體動脈與脈絡膜循環,因此其網膜血管屏障也應該是未受影響的。我們的結果顯示在低眼壓視網膜病變患者,無論是以玻璃體螢光測量儀或眼底螢光血管攝影檢查,其網膜血管屏障均未產生可測得的變化。

In order to study the blood-retinal barrier change of hypotony retinopathy, we collected seven eyes of six our glaucoma patients, who had received trabeculectomy with mitomycin-C soaking in recent four years. All these patients were suffered from hypotony retinopathy late after the operation for at least four months. The results of vitreous fluorophotometry and fluorescein angiography revealed normal blood-retinal barrier function on eyes with chorioretinal folds, macular swelling and retinal vessel tortuosity. However, one patient with marked bilateral discs edema showed prominent fluorescein stain on fluorescein angiography and increasing of posterior vitreous fluorescein penetrating value by fluorophotometry. The leakage might come from the disc circulation of posterior ciliary vessels and the blood-retinal barrier seemed not altered. The results of vitreous fluorophotometry and fluorescein angiography were compatible and we concluded that although the retina showed edematous appearance, the blood-retinal barrier function was not changed in hypotony retinopathy.
