復發性壞死性鞏膜炎之治療―病例報告
The Treatment of Recurrent Necrotizing Scleritis-A Case Report
摘要
在過去兩年半中，我們成功的以內科療法而非手術療法，治療了15例以上的壞死性鞏膜炎，其中多數是翼狀贅肉手術後引起，這15例的病例中，六成以上有綠膿稈菌陽性培養，且15例中除了一名壞死性鞏膜炎有復發外，其餘病人療程在二個星期至二個月不等，治療後觀察一個月至兩年不等，因其中14例均無再發現象，因此這名復發病例有必要加以討論並研究。在這位病人的治療過程中，我們以內科療法來治療之外，最後選擇病灶處即結膜最紅腫的部位，以結膜下注射Kenacort 0.5毫升來加速病情的改善。

In the past two and a half years, we successfully used medical methods rather than surgical procedures to treat more than 15 cases of necrotizing scieritis. None of the 15 patients except one had the tendency of recurrent necrotizing scieritis. The management of this patient was very challenging, but we also use medical method to treat this case. Finally this patient received subconjunctival injection of Kenacort 0.5cc. This patient had relief of her symptoms with clinically observable improvement of inflammation. No complications of scleral thinning or perforation ocurred in this patient.
