隱形眼鏡之認識及使用方法正確性之研究

摘要

在台灣並無合法之驗光師制度，所有眼鏡店從業人員均於缺乏任何任用資格或執照之情形下執行業務且多數眼鏡店均在不合法之情況下驗配隱形眼鏡。為瞭解此一問題對隱形眼鏡配戴者之影響，我們調查高雄醫學院194位配戴隱形眼鏡之學生，結果顯示使用軟式隱形眼鏡佔92%，硬式佔8%。84%的人在眼鏡公司驗配隱形眼鏡，而眼科醫師處只有24%。而選擇眼鏡公司驗配主要原因為方便(58%)及服務較佳(28%)。選擇眼科醫師驗配最主要原因是較專業(37%)及較有信心(28%)。從未留意隱形眼鏡包裝上之衛生署許可字號及此隱形眼鏡須向眼科醫師處方方能使用的有73.5%。未定期做隱形眼鏡及眼睛之檢查的有67%。82%的配戴者曾有不適症狀發生，其中48%的人會尋求醫師診治。72%的配戴者因戴隱形眼鏡較方便而配戴，但停戴原因中最高之比例則是因不方便而停用，佔44%，顯示配戴者在驗配之前對隱形眼鏡之認知不足。這是調查醫學院學生所得，對於一般社會大眾，其比例及百分比應會更高。這不僅暴露隱形眼鏡在商業宣傳促銷下所隱含的浮濫與危機，也充分顯示出使用隱形眼鏡者對驗配隱形眼鏡屬於醫療行為的陌生及使用衛教的生疏。因此，如何管理隱形眼鏡的合法驗配，提升正確配戴衛教認知，以減低配戴隱形眼鏡所引起之併發症，將是當前重要的課題。

There is no optometrist system in Taiwan. Most of the optician are practicing without any qualification. Despite of illegality, most of the optician fit contact lens for their customer. In order to evaluate the situation and influence of such kind of illegal fitting, we investigated 194 students of contact lens wearers in Kaohsiung Medical College. The results are, 92% of students wear soft contact lens, 8% wear hard lens which are all RGP lens. 84% of wearers have their contact lens fitted at optician, only 24% are fitted by ophthalmologists.
The main causes of choosing optician for their contact lens fitting are more convenience and better service. Those who were fitted by the ophthalmologists believe in the expertise and reliability. Although there are a short note including registered number of permission issued by the health department, and a statement that contact lens should be prescribed by an eye doctor on the package of the contact lens, 73.5% of wearers never noticed about this.
67% of wearers never receive routine examination. After contact lens fitting, 82% of wearers ever experience discomfort during the contact lens wearing, but only 48% of them consult the eye doctors for help. The majority of the reminders just wait and see whether the condition could improve spontaneously. An interesting finding is that 72% of the wearers wear contact lens because they believe that contact lens are more convenient. In contrast, among the students who ever cease the contact lens wearing, the main reason is incovenient. It shows that the wearers are lack of understanding of contact lens wearing before they fit the contact lens.
This is a survey among the medical college students, and we expect a worse condition among the general population. This survey reveals that most of the contact lens wearers are managed by illegal and unqualified optician. The risks of contact lens wearing are under-estimated. That contact lens fitting is a medical procedure is poor recognized. How to improve the education of contact lens wearers, to direct the wearers to the correct manipulation and to prevent the mal-prescription at the unqualified optician is a serious topics in Taiwan.
