以巨大圓頂狀深層網膜內出血為第一表徵之急性原髓細胞性白血病合併瀰漫性血管內凝血異常－病例報告
Huge Dome-Shaped Deep Intraretinal Hemorrhage as the Presenting Manifestation of Acute Promyelocytic Leukemia Associated with Disseminated Intravascular Coagulopathy-A Case Report
摘要
急性白血病的眼部侵犯可以發生在白血病程的各個時期，但以眼部症狀為第一表徵者較為少見。 我們報告一27歲無特殊病史之女性病例，以視力模糊為主訴症狀而至眼科急診，根據眼底出血形態及四肢瘀血等症候而臆斷為急性白血病合併瀰漫性血管內凝血異常。經血液檢查及骨髓穿刺確定診斷後，接受緊急輸血及全身性化學治療始得緩解。另外，此病例發生異常形態之網膜出血的機制也在本文中加以探討。

Ocular involvement of acute leukemia may be found at all stages of the disease, but it is uncommon that acute leukemia initially presents with ocular symptoms. We report a previously healthy 27-year-old female patient who came to our ophthalmic emergency room with the chief complaint of blurred vision. Acute leukemia associated with disseminated intravascular coagulopathy was impressed according to the characteristic appearance of retinal hemorrhage and ecchymosis of limbs. The diagnosis was confirmed by hematologic examination and bone marrow aspiration. Remission was established after urgent blood transfusion and systemic chemotherapy were given. Besides, the mechanism of the special appearance of retinal hemorrhage was also discussed in this article.
