巨細胞病毒視網膜炎－病例報告
Cytomegalovirus Retinitis after Liver Transplantation-Case Report
摘要
巨細胞病毒視網膜炎是在接受器官移植患者在術後最常見的伺機感染中的一種。本文報告一例肝臟移植後感染巨細胞病毒視網膜炎，一個54歲男性在肝移植術後3個月視力逐漸模糊，右眼可矯正至0.3，而左眼僅可矯正至0.2。細隙燈檢查發現雙眼有虹彩炎及玻璃體發炎，眼底檢查發現兩側均有出血，血管附近有界限分明的視網膜壞死，及壞死區上的血管鞘膜化，眼底螢光攝影在對應於眼底病變處有螢光滲漏。血液檢查發現有白血球低下及巨細胞病毒抗體呈陽性反應，其症狀在接受ganciclovir治療後逐漸改善。

Cytomegalovirus (CMV) retinitis is one of the most common ocular opportunistic infection in transplant recipient. We presented a patient of CMV retinitis in both eyes after liver transplantation. A 54-year-old male complained of gradually blurred vision in both eyes at 3 months after liver transplantation. His best-corrected visual acuity was 0.3 in the right eye and 0.2 in the left eye. Slit-lamp examination demonstrated anterior uveitis and vitritis in both eyes. Examination of both fundi revealed hemorrhage, well-demarcated areas of retinal whitening in a perivascular distribution, and vascular sheathing in the areas of whitening. Fluorescein angiography showed mild fluorescein dye leakage at the corresponding areas of the lesions. Blood examinations revealed leukopenia and positive IgM against the CMV. Symptoms and signs improved after treatment with ganciclovir.
