圓錐角膜的彩色地形圖分析

Detecting Keratoconus with Videokeratography Eyesys Technologies Version 4.0

摘要


為了尋求初期圓錐角膜快速而有效的診斷方法並篩選屈光手術適應者,我們收集過去10年內21隻臨床診斷為keratoconus及158隻非keratoconus眼睛的角膜地形圖,並採用本院既有的角膜地圖儀EyeSys Technologies Version 4.0的Holladay Diagnostic Summary當中兩個參數:Eff RP(Effective refractive power),I-S value (inferior-superior value)做為本試驗的診斷工具,診斷Keratoconus的標準為Eff RP及I-S value大於正常眼的平均值+二個標準差,計算所得的敏感度(test A:100% test B:81%)及明確度(test A:87% test B:97%),不但可以幫助圓錐角膜的鑑別診斷,還可有助於屈光手術的術前評估,降低術後屈光不穩定的可能性。

Twenty-one eyes with keratoconus were selected from our clinic in the past 10 years for studying the quantitative approaches to identify the videokeratographic information of keratoconus by EyeSys Technologies Version 4.0. We used effective refractive power (Eff RP) and the inferosuperior asymmetry (I-S value) (parameters at the Holladay Diagnostic Summary) as indicators to quantify the degree of topographic abnormality in corneas with keratoconus. Criterias for diagnosis were chosen as normal values plus 2 standard deviations. Video-keratographs of clinically diagnosed keratoconus (n=21) and of various non-keratoconus conditions (n=158, including normal, with-the-rule astigmatism, post penetrating keratoplasty, pterygium, contact lens inducedcorneal warpage……) were selected. Sensitivity and specificity were then calculated. Our study results of high sensitivity (test A:100%, test B:81%) and high specificity (test A:87%, test B:97%) suggest a comparable useful and beneficial method in detection of keratoconus pattern for screening candidates for refractive surgery.
