乳癌轉移到眼眶及眼瞼的腫瘤－病例報告
Breast Carcinoma with Intraocular and Intraorbital Metastasis-A Case Report
摘要
本文報告一位七十六歲的婦女，近一年多來左眼上眼瞼出現紅腫的硬塊，其外表看似眼瞼炎，所以初診時被誤以為眼瞼發炎來治療，但始終無法治癒；後來轉到三軍總醫院眼科，其時左眼上眼瞼已變為完全下垂不能上下活動，並且眼瞼成為硬塊狀態，眼球也出現內陷；病人曾於二十幾年前因乳癌做過乳房切除術，此時該患者經眼部電腦斷層檢查及組織病理切片初步診斷為轉移性眼眶及眼瞼腫瘤。再進一步使用免疫組織化學染色，檢定腫瘤細胞內含有雌激素受體；因此，確定診斷本病例為乳癌細胞轉移形成的腫瘤。

We report a case of breast carcinoma with both intraocular and intraorbital metastasis. A 76-year-old female was referred with left eyelid induration, motionless, ptosis and enophthalmos. She had been managed under the impression of eyelid chalazion at the local clinics for more than one year. She also suffered from right side breast cancer, received right radical mastectomy about twenty years ago. Orbital CT revealed that tumor mass with intraocular and intraorbital extension. Tissue biopsy with immunohistochemical stains proved to be breast adenocarcinoma with high estrogen activity. Finally, she received local radiotherapy and oral hormone therapy.
