目的:高度近視可能是網膜剝離的危險因子之一,本篇的目的主要探討週邊網膜退化和屈光度之關係。 方法:這是一個回溯性的研究,我們收集台大眼科門診由1995年至2000年曾接受週邊雷射光凝固術並排除糖尿病網膜病變、分枝靜脈阻塞或其他網膜血管疾患之病人,共1868眼(934例病人)。經完整的眼科檢查及眼底檢查並詳細記錄週邊視網膜變化,然後分析這些週邊視網膜變性與病人屈光度及年齡之相關性。 結果:總體而言,蝸牛紋狀退化、格子狀退化、萎縮性裂孔、懸垂物的發生率在近視眼較高。而翼狀裂孔、局限性視網膜剝離和玻璃體牽扯力則和近視沒什麼相關。蝸牛紋狀退化和玻璃體牽扯力較易產生於年輕人,而其他的週邊網膜退化則和年齡無關。 結論:預防近視可以減低週邊視網膜退化及視網膜剝離,除此之外,定期仔細的眼底檢查是預防視網膜剝離最重要的方法。

Purpose: High myopia might be one of the risk factors of retinal detachment. The aim of this study is to explore the relationship between the peripheral retinal degenerations and refractive errors. Methods: This is a retrospective study. We collected 1868 eyes in 934 patients who underwent peripheral retinal photocoagulation during 1995 to 2000 at outpatient clinic of the Department of Ophthalmology, National Taiwan University Hospital. The patients received photocoagulation for diabetic retinopathy, branch retinal vein occlusion or other retinal vascular disorders were all excluded from this study. After complete ocular examinations, the peripheral chorioretinal lesions were recorded. The refractive status was recorded as the spherical equivalent. The data were analyzed for the relationship between the peripheral chorioretinal lesions and patient's refractive error, gender and age. Results: Overall, the incidence of snail track degeneration, lattice degeneration, atrophic hole and tag are higher in myopic eye. Flap tear, localized retinal detachment and vitreous traction don't have such relation with myopia. Snail track degeneration and vitreous traction were more frequently found in younger patients. Other peripheral chorioretinal lesions did not show such relation with age. Conclusions: Prevention of myopia might lessen the incidence of peripheral chorioretinal lesions and retinal detachment. Besides, most important method to prevent the retinal detachment is to receive detailed fundus examination routinely.
