Retinal Detachment in Chorioretinal Coloboma-A Case Report
網膜脈絡膜缺損引起之網膜剝離
摘要
本文報告在一位16歲右眼視網膜脈絡膜缺損之男性身上，所出現之裂孔性視網膜剝離表現。此病人除了右眼視網膜脈絡膜呈現缺損，右眼之虹膜亦於鼻下側方向有鑰匙孔狀缺損。右眼之視網膜裂孔位於罕見之視網膜缺損區邊緣，亦即坐落於正常與缺損視網膜交界處，其所形成之視網膜剝離，沿此缺損邊界，造成正常之視網膜區全數剝離，而未波及視網膜缺損區。此處將報告手術方式，包含經坦部玻璃體切除術合併鞏膜扣壓術，氣體填充術等等。術後此病人之玻璃體內氣體吸收之後，視網膜仍維持貼著狀態。

Purpose: To report a case of rhegmatogenous retinal detachment in a chorioretinal colobomatous patient, in whom the retina detached along the coloboma border through a slit-like break just at the junction of coloboma and normal retinal tissue. Method/Participants: Reporting one case with iris coloboma and a chorioretinal coloboma in his right eye which was noted incidentally during operation for his spontaneous retinal detachment. Results: Retina was reattached after pars plana vitrectomy combined with scleral buckling procedure and gas tamponade. Conclusion: A break at the junction of coloboma and normal retina tissue can cause a rhegmatogenous retinal detachment outside rather than inside the coloboma. The break can be well-sealed and retina reattached successfully by pars plana vitrectomy combined with scleral buckling and gas tamponade.
