LASIK手術病人中心角膜厚度以Pentacam OrbscanⅡ及超音波厚度儀之測量結果

Central Corneal Thickness Measurements with Pentacam, Orbscan Ⅱ and Ultrasound Pachymetry Before and After Lasik

摘要


目的:針對接受LASIK的病患,於手術前、後分別用Pentacam、Orbscan Ⅱ、及超音波(ultrasound pachymetry)測量中心角膜厚度,來比較三種儀器之間的異同。 方法:本研究共收集LASIK手術前31位病人(62隻眼睛)、及LASIK術後四週32位病人(64隻眼睛)的資料,依序用Pentacam、Orbscan Ⅱ、及超音波測量中心角膜厚度,將三種儀器的測量結果作ANOVA統計分析。結果:LASIK手術前三種儀器的測量結果呈現高度相關,三者間並無差異;LASIK術後四週,三種儀器之間的差異有統計學上的意義,其中以Orbscan Ⅱ所量的厚度最薄(408.15±46.19μm)、Pentacam最厚(437.15±34.46 μm)。 結論:正常角膜用Pentacam、Orbscan Ⅱ、及超音波測量中心角膜厚度,結果相似,也都有極佳的可重複性(repeatability)及正確性(validity);但是,在LASIK術後的病人,用Orbscan Ⅱ測量的中心角膜厚度則明顯比超音波及Pentacam要薄。所以,在LASIK術後,Pentacam可做為超音波以外,測量中心角膜厚度的參考,但若用Orbscan Ⅱ測量時,則要特別留意低估角膜厚度的情形。

Purpose: The present study was designed to compare Pentacam, Orbscan Ⅱ, and ultrasound pachymetry in measuring central corneal thickness (CCT) in eyes before and after LASIK surgery. Method: A total of 31 patients (62 eyes) were measured before surgery. Another group of 32 patients (64 eyes) were measured 4 weeks after LASIK. The measurements of central corneal thickness were performed in the sequence of Pentacam, Orbscan Ⅱ, and ultrasound pachymetry. The data were collected and analyzed with ANOVA. Result: Before surgery, the CCT measured by 3 modalities were not significantly different (p=0.136). However, statistical difference was found for CCT measurements between 3 instruments after LASIK (p<0.001). Besides, CCT values obtained by Orbscan Ⅱ were much thinner than those obtained by Pentacam and ultrasound pachymetry. Conclusion: Pentacam, Orbscan Ⅱ and ultrasound pachymetry obtain similar CCT values in normal eyes, while Orbscan Ⅱ obtains thinner CCT values than Pentacam and ultrasound in eyes after LASIK. This discrepancy may lead the surgeon to be more conservative in using Orbscan Ⅱ after primary refractive surgery.
