因視網膜出血以及視神經水腫的症狀而發現的慢性骨髓性白血病病患:病例報告

Retinal Hemorrhage and Optic Disc Edema as Initial Clinical Problems of Chronic Myeloid Leukemia: A Case Report

摘要


目的:報告一名因雙眼視網膜出血及視神經水腫引起的視力障礙為初始症狀,而診斷罹患慢性骨髓性白血病之病患。方法:病例報告。結果:32歲男性,因雙眼視網膜在半年內陸續發生出血現象轉診至本院。發病初期只有右眼病變,因患者有近視而被臆測為近視併發症,經過半年後左眼也發生同樣症狀,出血範圍不僅只有黃斑部視網膜而且合併有視神經水腫現象。檢查結果發現為慢性骨髓性白血病。經過化療以後,患者的視網膜出血狀況以及視力已經逐漸改善,目前仍在門診追蹤中。結論:白血病造成眼球的併發症為多樣性,以視網膜出血較為常見。對於高度近視的年輕患者發生黃斑部出血並且有不尋常之身體異常病史時,需與其它可能病因做鑑別診斷。

Purpose: To report a case of chronic myeloid leukemia with retinal hemorrhage and optic disc edema as initial problems.Methods: Case report.Results: A 32-year-old male suffered from initial right eye blurred vision for 6 months. Because of high myopia, related macular hemorrhage was suspected and the patient was kept under observation without aggressive evaluation until his left eye showed the same problem. Bilateral multiple retinal hemorrhages with mild optic disc edema were noted. Laboratory examination revealed marked leukocytosis and chronic myeloid leukemia was confirmed. After chemotherapy, his general condition was stable and the ocular problems gradually regressed.Conclusions: Leukemia may present many ocular manifestations. For young people with high myopia and unusual systemic problems, aggressive evaluation and differential diagnosis are strongly suggested.
