Changes of Regional Cerebral Blood Flow Detected By HMPAO Brain SPECT in Creutzfeldt-Jakob Disease

摘要


前言:本研究之目的在於比較使用鎝-99m-HMPAO單光子電腦斷層攝影(99mTc-HMPAO SPECT)、電腦斷層(CT)、磁振造影(MRI)及腦電波圖(EEG)來偵测Creutzfeldt-Jakob病(CJD)之腦病變。方法:14位CJD病患接受鎝-99m-HMPAO SPECT,CT,MRI及EEG來偵測大腦皮質及基底核並比較其結果。結果:鎝-99m-HMPAO SPECT顯示所有14位CJD病患於大腦皮質及基底核皆有多發、廣泛且不規則腦血流減少現象。8位病患之CT或MRI僅顯示不明顯之腦萎縮現象。14位病患之EEG皆有異常之腦電波(PLEDs)於兩側大腦半球。結論:本研究結果顯示若與CT或MRI比較,鎝-99m-HMPAO SPECT是一種偵測CJD病患腦病變比較靈敏的工具。

Background: Our purpose was to detect brain lesionsin Creutzfeldt-Jakob disease(CJD)using brain technetium-99m hexamethyl-propyleneamineoxime(99mTc-HMPAO)imaging in comparison with computed tomography(CT),magnetic resonance imaging(MRI)and electroencephalography(EEG).Methods:99mTc-HMPAO brain images, using fan-beam single-photon emission computed tomography(SPECT),were used to detect cerebral cortex and basal ganglion anomalies in CJD. Fourteen CJD patients received brain 99mTc-HMPAO imaging, and CT,MRI and EEG were also performed for comparison.Results:99mTc-HMPAO brain images demonstrated multiple diffuse and irregular hypoperfusion areas over the cerebral cortices and basal ganglia in all 14patients.In 8 of 14 cases, brain CT or MRI showed only unremarkable aged brain atrophy. EEG revealed periodic lateralizing epileptic form discharges(PLEDs)over the bilateral hemispheres in all 14 patients. Conclusion: This study suggests that, in comparison with brain CT or MRI,99mTc-HMPAO brain imaging is amore sensitive tool for the detection of brain anomalies in CJD patients.
