肝癌病人骨掃描時鎝-99m-MDP聚集於腎臟皮層之探討

Increased Uptake of 99mTc-MDP by Renal Cortex in Patients with Hepatoma

摘要


前言:我們曾報告鎝-99m-MDP在重症之肝疾病患者骨掃描時會有聚集於腎皮層增加之現象,並推測可能與重症之肝疾病併發腎症候(hepatorenal synodrome)有關。本文目的即再深入探討肝癌病人骨掃描時鎝-99m-MDP聚集於腎皮層增加之原因。材料與方法:回溯最近一年內所有骨掃描病例,並自其中收集有鎝-99m-MDP聚集於腎皮層增加且有肝癌之比例,就其臨床資料與檢驗數據作分析。結果:8例有鎝-99m-MDP聚集於腎皮層增加之肝癌病人中,有血中白蛋白濃度偏低之情形及可能者有7例。其中曾接受放射治療一例、併有高血鈣一例,腎功能異常者有二例。結論:骨掃描時鎝-99m-MDP聚集於腎皮層增加之肝癌病人,與病人血中白蛋白濃度偏低有極大之相關。

Background: We previously reported increased uptake of 99mTc-MDP by renal cortex in a patient with advanced hepatic disease and oliguria. It is suggested that hepatorenal syndrome may be the cause. To clarify this, we investigated the mechanism of the increased uptake in the patients with hepatoma who underwent 99mTc-MDP bone scanning.Materials and Methods: We retrospectively studied all the bone scans undertaken since one year ago. All those with increased renal uptake of 99mTc-MDP during bone scanning and with diagnosis of hepatoma were selected. Clinical history and laboratory data were studied.Results: Among 8 cases with increased renal uptake of 99mTc-MDP and hepatoma, 7 were noted to be with decreased serum albumin level. In these 7 patients, one underwent radiotherapy, one was with hpercalcemia and two had abnormal BUN and creatinine.Conclusion: Increased uptake of 99mTc-MDP by renal cortex in the patients with hyepatoma is very likely to be related with the patients' decreased serum albumin level.
