A False-Positive Radioiodine-131 Whole-Body Scan in Thyroid Carcinoma Caused by Menstruation Localized on SPECT/CT

摘要


碘-131甲狀腺癌全身掃描的處置方法,己常規使用於經接受全甲狀腺切除或近全甲狀腺切除之分化良好的甲狀腺癌患者。近期,附加電腦斷層的單光子放射電腦斷層掃描之應用已經逐漸增加,並產生了理想的結果。文獻上曾報導引起假陽性碘-131掃描的各種原因。我們報告一例假陽性的病例,發現因月經積累造成子宮中央部分,仿如甲狀腺癌轉移至骨盆腔,附加電腦斷層的單光子放射電腦斷層掃描與臨床病史證實了這一點。

Radioiodine whole-body scintigraphy is indispensable for the management of patients with well-differentiated thyroid carcinoma who had undergone total or near-total thyroidectomy. Recently, the additional imaging of SPECT/CT has been increasingly utilized for more optimal interpretation. Previous literatures reported various causes of false-positive findings on radioiodine scan. We report a false-positive study from radioiodine-131 accumulation in the uterus during menstruation, mimicking a pelvic metastasis. This was subsequently confirmed by clinical history and SPECT/CT.
