Incidental Finding of Brain Metastasis on SPECT/CT in a Thyroid Cancer Patient

Radioiodine whole-body scintigraphy is indispensable for the management of patients with well-differentiated thyroid carcinoma, who had undergone total or near-total thyroidectomy. Recently, the additional imaging of SPECT/CT has been increasingly utilized for optimal interpretation. Previous literatures reported SPECT/CT had incremental diagnostic values compared to planar scintigraphy on therapeutic management of thyroid cancer. We present a case of a thyroid cancer with right parietal brain metastasis detected on SPECT/CT which is negative on the 3-mCi diagnostic ^(131)I whole body scan. To the best of our knowledge, there’s neither a similar case of brain metastasis with a positive ^(131)I SPECT/CT nor has a negative planar image in a patient with well-differentiated thyroid carcinoma been reported in the literature.
