Severe Tricuspid Regurgitation and Right Side Heart Failure as Major Manifestations of Chronic Pulmonary Embolism: A Case Report

嚴重三尖瓣逆流及右側心臟衰竭源發於慢性肺栓塞及下肢深靜脈栓塞的臨床表現：一病例報告

摘要

一45歲之女性病患，臨床表現為輕微運動時呼吸困難及下肢水腫持續兩年。病人在理學檢查上看到明顯的頸靜脈鼓賬，無呼吸雜音，在左側胸骨柄下緣聽到明顯的心臟全收縮期雜音。病人在室溫下周邊動脈血氧分壓為57mmHg。病人接受雙相及彩色杜普勒心臟超音波檢查顯示肺動脈高壓，嚴重的三尖辦逆流，右心房及右心室擴大，正常的左，右心室收縮。肺部核子醫學掃瞄顯示雙側肺葉組織多處血液流動障礙，肺葉通氣核子醫學檢查正常。病人的肺栓塞經由下肢靜脈血管攝影證實來自下肢靜脈血栓。病人最後診斷為慢性複發性肺栓塞合併重度三尖辦閉鎖不全及右心衰竭。

We report a case of chronic recurrent pulmonary embolism, who presented with pulmonary hypertension, severe tricuspid regurgitation and right sided heart failure. The source of pulmonary emboli was found to be deep vein thrombosis of both lower extremities which was confirmed by the lower limbs' venograhy. The clinical characteristics, radiographic features, and echocardiographic findings along with Tc-99M macroaggregated albumin lung perfusion scan and Xe-133 ventilation lung scan are presented. These findings proved the diagnosis of chronic pulmonary embolism with severe tricuspid regurgitation and right-sided heart failure.
