Introduction: Malaria is caused by protozoan parasites Plasmodium that infected red blood cells. Non-immune travelers are particularly at high risk for severe malaria. Fever in a traveler within 3 months of possible exposure should prompt immediate investigation of the disease. Case reports: We presented a case who is a photographer traveled back from malaria endemic area and developed severe malaria with the presentation of high fever, consciousness change and severe jaundice. The sepsis developed and deteriorated rapidly, the intensive care therapy started immediately. The patient was fully recovered under treatment with Artesunate. The presentation, diagnosis, and treatment guidelines were discussed. Conclusion: People with fever and relevant epidemiologic exposure due to occupational reasons should workup for malaria immediately. Currently parenteral artesunate is the drug of choice for severe malaria. With adequate intensive care, patients with severe malaria could be recovered well.
