台灣交通意外事故歷年來居高不下,揮鞭症候群(Whiplash syndrome)則是車禍後最常伴隨的傷害之一。揮鞭症候群是指因某意外事故,導致頭頸部因慣性造成不正常的過度拉扯,對於肌肉、韌帶等結締組織所造成的損傷,在車輛追撞事故中尤其常見。除了急性期的頸部疼痛與僵硬等症狀,近40%的個案可能演變為慢性頸部疼痛,嚴重者還可能伴隨頸椎骨折、椎間盤突出或頸椎神經根壓迫症等等併發症,對於患者的生活品質與工作能力造成極大損害。本文藉由一位貨車司機遭受大型車輛追撞後,初期產生頸部痠痛、頭暈等症狀,後續發現第五、第六節頸椎椎間盤突出合併神經壓迫,進而懷疑為揮鞭症候群併發症的案例討論,進行相關文獻回顧,探討如何針對車輛安全配備及座椅的設計進行傷害預防,最後比較國內外現行車輛安全法規趨勢,以期大眾能對車輛安全的重要性有更深入的了解,減少職業駕駛人意外傷害的發生。

The traffic accidents in Taiwan remain a serious social-economical issue. Whiplash syndrome is considered as one of the main cause of traffic injuries. It is a neck injury due to rapid back-and-forth movement of the neck leading to sudden distortion and damage, which most often occurs during a rear-end vehicle accident. Aside from the acute-phase symptoms including neck pain and stiffness, near 40% of the victims may develop chronic neck pain. Serious complications comprise fracture, disc herniation or nerve root compression, which bring great loss to the patient’s life quality and work capacity. We presented a light truck driver experiencing neck soreness and dizziness after suffering from rear-end collision by another large vesicle then developed C5-C6 disc herniation with spinal cord compression. Whiplash syndrome was highly suspected according to his history. Thus, we conducted literatures review and aimed to investigate the vehicle safety equipment and seat designs. We also compared the current Taiwanese vehicle safety regulations with other countries and we expect to raise the public perception for the importance of vehicle safety and prevention of accident injuries to professional drivers.
