Lobar consolidation of cryptococcal pneumonia is rarely reported in immunocompetent hosts. We report a healthy 25-year-old woman who presented with pneumonia in her left lower lobe. A poor response to empiric antibiotics was noted, and a subsequent investigation disclosed high titers of serum cryptococcal antigen (CSA titer >1:1024). Gomori-methenamine silver staining of bronchoalveolar lavage fluid disclosed cryptococci with a cryptococcal antigen titer of 1:128. The patient's symptoms improved with resolution of the consolidation of the left lower lobe of the lung after treatment with fluconazole. We also reviewed the literature.
