Pneumonia is a known cause of pediatric abdominal pain and its usual location is in the right lower lobe. Contrary to common belief, we have found one case of pneumococcus lobar pneumonia situated in the right upper lobe with the initial presentation of fever, intermittent abdominal pain, just very mild cough, and no rales or rhonchi on physical examination. This case soon went down-hill to impending septic shock with disseminated intravascular coagulation. But she recovered quite smoothly after high dose penicillin was given.
