台灣質子加速器治癌設備之規劃與建立

Planning and Establishment of Proton Therapy Facility in Taiwan

摘要


惡性腫瘤是台灣地區十年來十大死亡原因的首位,其趨勢似有更嚴重而無好轉徵象,對民眾的健康、生命與國力的損耗甚大,急需有所突破。質子射線治療惡性腫瘤經世界各國十餘個加速器治療研究所一萬一千餘例臨床試驗,證實不但對腫瘤的控制率和癌患存活率可大幅度提高,同時治療的副作用則相對降低甚多。新的加速器建造和治療科技的進步,近年來已自研究用加速器兼做癌病治療轉型為建造醫院專用性的質子加速器治療機,符合人體工學和醫、護、技人員的要求,不但使每天治療病人的效率大增,同時精準度高。病人也較舒適與安全。然而醫院專用質子加速器治療機的構造複雜且精密,初始投資昂貴,需要配合的物理、生物和醫學研究的項目也很多,如何評估加速器治療機的部分或全部在國內設計與建造,宜由海內外學人共組研究群,從事可行性研究,提出整體計畫與預算,並向相關單位提出短、中期設計、施工、使用、維修和研究等分項研究計畫。建議行政院作跨部會的策畫與領導。本計畫一旦落實推動,不但造福癌患,同時將加速器有關設計、建造、維修、使用與研究的尖端科技人才推展到國際第一流的水準,對國內學術的提升、國家形象的增進及國民的福祉都大有俾益。

Cancer is the number one killer in Taiwan since 1982, the trend of cancer mortality has no signs of reverse but is getting worse, the impact of cancer on the population health, life span and the national strength are tremendous that certain break through is mandatory. Radiotherapy of malignant neoplasms with proton beams has been practicing in at least 11 institutes worldwide and has accumulated experience for more than 12,000 patients, the tumor control rate and survival rate of cancer patients are much better than conventional radiotherapy while the complication rate is remarkably lower. Owing to the advancement of dosign and constrution of accelerator as well as new technology in cancer patient management, hospital based proton therapy machine is the future trend of development which will be better and convenient to the clinical needs than to use the research proton facility for cancer treatment. Accordingly, one dedicated proton machine can treat large number of cancer patients per day under highly accurate, comfortable and safe condition. However, hospital based proton accelerator is a very complicate facility involving a lot of advanced high technology. The initial investment is high, it needs keen collaboration of research and dovelopment between physics, biology, engineering and medicine. It seems advisable to organize a task force from local and overseas experts in this field to do a feasibility study, evaluate the capability and the merit to build a part or the whole facility by the local scientists and engineers, submit the proposals regarding design, construction, application, maintenance and research of proton therapy facility, recommend the Executive Yeun to lead an interdepartmental group to promote this project, so that it would not only benefit to the cancer patients, but allow the development of the related sciences and technology of proton accelerator into the most advanced international standards, consequently elevate the academic level and the national pride and to promote the welfare for the people in this country.
