Combined Therapy of Surgical Excision and Electron Beam Irradiation for the Treatment of Keloid

合併利用外科切除術及電子光束治療蟹足腫

摘要


在蟹足腫的治療與預防上,合併外科切除及電子光束照射療效不錯,但缺乏長期追蹤報告。台中榮總自1997年8月至1998年12月,共在41個病人身上,43處蟹足腫,予以切除後再以電子光束治療。初期成效不錯,療效達74.4%。我們也發現某些區域有容易再發的情形。我們並且討論治療結果可能原因及作文獻回顧。總而言之,治療蟹足腫可合併數種療法,且有一定的療效;但仍須長期追蹤治療,方可將其復發率減至最低。

We prospectively evaluated the response to surgical excision followed by electron beam irradiation of 43 sites of keloid in 41 patients followed up for an average of 19 months. All patients followed the same protocol: keloidectomy, sutured with 2 layers, and electron beam irradiation. After a follow-up phone call and OPD visit to each of the patients, the response rate was 74.4%. However, there was a tendency for the keloid to recur. There was a statistical significance between responses A & B and response C (17.8±7.6 vs.23.6±6.2 months, p=0.029). There was also a regional difference in response. Combined therapy of surgical excision and electron beam irradiation is fast and the results are fair. Combined therapy of surgical excision, electron beam irradiation and steroid infiltration is probably more effective than any other single treatment modality in short period. But long-term results may require further investigation under more strict study design.
