摘要

在處理手部壓砸傷的病人時，指甲缺失是一個很難解決的問題。指甲可以提供指尖支撐、觸摸的反作用力、以及美觀。尤其對女性來說，指甲缺失是一件很難忍受的事。文獻上指甲重建可分三類：人工指甲、不經血管吻合手術的游離指甲瓣、及經血管吻合手術的游離指甲瓣。在第三類中，不僅較符合生理需求，而且較不會有萎縮現象發生。這樣的手術也在改良後變得較易施行。
我們提出本身的經驗，這些病人本身對手術結果都相當滿意，也符合期望。在手部遭壓砸傷而造成指甲缺失的病人，經血管吻合手術的游離指甲瓣為一個在重建時非常值得攷慮的選擇。

Nail missing is a difficult problem in dealing with the patients who had crushed injury of hand. Nail can provide the fingertip with support, counter-pressure for touch and also a cosmetic outlook. Especially in women, nail loss in any digit becomes a major concern. Among the three methods of nail reconstruction, microsurgical nail transfer is improving than ever. We report two cases, which received free venous nail flap transfer as solution to their hand crushing injury. Both were females. The first patient has the nail transfer on right middle finger, and the other is on right little finger. Their hand crushing injury was caused by the crushed injury of window and machine. Both nail flaps survived. The patients were satisfied with the results. Microvascular free nail flaps were a choice for nail reconstruction.
